The
suspension of micronized Chromoglycate and propellants inside the
container of the Spray Laríngeo Chromoglycate, together with the
nasopharyngeal cannula, originated this brand new therapeutic product
that permits the direct application and dosage on target organs in
lower respiratory airways in sports equines.
Spray
Laríngeo Chromoglycate inhibits both the immediate and non-immediate
bronchoconstrictive reactions to inhaled antigen. It prevents bronchial
hyper-reactivity and bronchospasm caused by exercise, toluene
diisocyanate, aspirin, cold air, sulfur dioxide, and environmental
pollutants.
Horses
with lower respiratory airway inflammatory ailments: acute bronchial
obstruction, SAID, RAD, COPD (chronic obstructive pulmonary diseases),
EIPH (exercise induced pulmonare hemorrhage) or any of these diseases can keep on training with the application of Spray Laríngeo Chromoglycate by means of a flexible nasopharyngeal cannula.
Spray
Laríngeo Chromoglycate is a desensitizer and inhibitor of the
mechanisms that activate the bronchoconstrictive hyper-reaction that
occur in equines presenting SAID, RAD, COPD (chronic obstructive
pulmonary disease) including the exercise-induced bronchoconstriction,
EIPH (exercise induced pulmonary hemorrhages) and other actions caused
by allergens. It does not perform in the acute process, but after 3 to
4 weeks' treatment (twice a day) a desensitization is reached that
lasts 4 days after withdrawal.
These
sprayings are applied directly in the pharyngeal cavity with a flexible
cannula through the nasal passages. The pharynx acts as an expansion
chamber for the big drops to deposit thus permitting drops smaller than
3 microns stay in suspension be carried by the inspiratory flux into
the lungs. This spray can also be applied intra-tracheally using a
special needle accessory.
Spray
Laríngeo Chromoglycate, due to its propelling force with fine drops, is
the only device that allows the exact application and dosage on the
bronchi and bronchioles. It has no intrinsic bronchodilator or
antihistamine activity.
EASY TO USE
Shake
the canister energetically. Introduce the cannula forward through the
ventral meatus of the nasal cavity until reaching the pharynx
(expansion zone), attach the canister, extend the horse's head, close
the nostrils briefly to provoke a slight inspiration, let go and press
for 2 seconds (one dose). Press again for another 2 seconds (second
dose) with the next inspiration. This treatment must be performed in
the morning and in the afternoon to reach the necessary desensitization
and prevention.
FORMULA:
Micronized Sodium Chromoglycate 2.4 g
Excipients and propellants 65 g
THERAPEUTIC ACTION:
Spray
Laríngeo Chromoglycate, when administered beforehand, acts as a
non-corticoid anti-inflammatory that prevents mastcell degranulation.
In allergic reactions and bronchial inflammation, it inhibits the
release of mediators that trigger bronchial hyper-reactivity. A
long-term application (3-4 weeks) helps control and prevent acute
bronchial obstruction induced by exercise and produced by exposure to
unspecific irritants that inhibit bronchial hyper-reactivity that cause
bronchoconstriction. After withdrawal, the effect lasts for 3-4 days,
notoriously improving breathing.
INDICATIONS:
Especially indicated in equines with a tendency for acute bronchial
hyper-reactivity, SAID,RAD, COPD (chronic obstructive pulmonary
disease), EIPH (exercise induced pulmonary hemorrhage), pulmonary
emphysema, bronchoconstriction induced by exercise and other allergens
that produce acute and chronic bronchial obstruction.
DOSAGE & ADMINISTRATION:
Introduce
the cannula through the ventral meatus of the nasal cavity, into the
pharynx. Close the nostrils to make the horse hold its breath for a
short while, to induce a slight profound inspiration. Release, and when
inspiration starts, push for 2 seconds, delivering a 40 mg. dose. Shake
again and repeat the procedure in the next inspiration for 2 more
seconds, thus completing the recommended 80 mg dose, twice a day
(morning and afternoon) regularly for 3 to 4 weeks minimum.
After withdrawal, the desensitizing effect lasts for 4 days.
The
pharynx and the trachea are used as an expansion chamber and deposit of
the bigger particles, thus leaving those particles smaller than 3
microns in suspension, diminishing resistance (60%) from the upper
respiratory airways, carrying the particles deep into the lungs by the
inspiratory flux. Long term use significantly reduces bronchial
hyper-reactivity that produces acute bronchial obstruction.
INTRATRACHEAL ACCESSORY:
If using
the special intra-tracheal needle adapter, introduce the needle in any
intercartilaginous area of the trachea with a 21 gauge x 1". The needle
must be kept longitudinal to the trachea. Shake energetically and
attach the Spray Laríngeo Chromoglycate canister (always vertical, the
arrow pointing upwards) to the adapter. This featured accessory is for
exclusive use by Veterinarians and is sold separately.
NOTICE FOR THE VETERINARIAN:
When
applied for the first time, Chromoglycate may produce
bronchoconstriction, therefore, in this case, we strongly recommend
that a dose of Spray Laríngeo Salbutamol be given beforehand.
PRECAUTIONS:
The
cannula and the adapter for the intra-tracheal needle must be cleaned
using warm water, making it flow inside by means of a 21 gauge needle,
after each application. Afterwards, shake the cannula to remove the
excess water and leave to dry.
If
blockage due to accumulation of Chromoglycate occurs, the spray will
fail because the drug will not be delivered correctly in the accurate
dose.
After use, proceed to disinfect the cannula/needle adapter.
Do not
use the same cannula on different horses. Disinfect after each use.
When taking the needle out of the trachea (a septic means) some
microorganisms may deposit on the subcutaneous tissues.
THE
FOLLOWING STEPS MUST BE FOLLOWED WHEN USING SPRAY LARINGEO
CHROMOGLYCATE IN ORDER TO MAKE CERTAIN THAT THE CORRECT DOSAGE IS
APPLIED. IT IS VERY IMPORTANT THAT THESE STEPS BE TAKEN TO INSURE THAT
THE CHROMOGLYCATE REACHES THE BRONCHIOLES.
- Spray
Laríngeo Chromoglycate must be applied with the canister always held in
upright vertical position (not inclined), with arrow on label pointing
up.
- Shake Laryngeal Spray Chromoglycate energetically.
- Apply twitch to upper lip.
- Place the head of the horse in the same line as the neck.
- Connect the cannula to the canister.
- Introduce half the cannula through one of the nostrils.
- Cover nostrils to stop breathing for a short period of time in order to induce a slightly profound inspiration.
- Shake again and introduce the cannula completely.
- Release nostrils and when breathing begins, press pushbutton for 2 seconds, delivering a 40 mg. dose.
- Extract the cannula halfway and shake again.
- Reintroduce
the cannula completely and apply 2 more seconds during the next
inspiration, thus completing the dosage with a total of 80 mg.
The
cannula and the adapter for the intra-tracheal needle must be washed
through with a green cone needle (21 g) after use. Shake the cannula to
remove excess water and leave out to dry, then disinfect.
The
blockage caused by accumulation of medication could cause an incorrect
dosage, therefore, it is important to keep the cannula and
intra-tracheal adapter needle clean.
